Requirements include the following documentation

Obtainable from FEM:

  • Completed FEM application forms
  • Completed Record of Information
  • Signed and dated POPIA clauses document

Supplied by the Applicant:

  • Company registration documents (Certificate of Confirmation / CK1 or Cor14.3)
  • Copy of the director’s / owner’s ID documents, as per the CK1 documents
  • Letter from the bank (not older than 3 months) confirming banking details
  • On a letterhead:
    – Confirmation of nature of business
    – Inception date of operations (as per current underwriting year)
    – Confirmation that there have been no accidents since inception

EXISTING COMPANIES

Transferring from the Compensation Fund
Requirements include the following documentation

Obtainable from FEM:

  • Completed FEM application forms
  • Completed Record of Information
  • Signed and dated POPIA clauses document
  • Final CF Return of Earnings

Supplied by the applicant:

  • Company registration documents (Certificate of Confirmation / CK1 or Cor14.3)
  • Copy of the director’s / owner’s ID documents, as per the CK1 documents
  • Letter from the bank (not older than 3 months) confirming banking details
  • Confirmation of wages/salaries/ payroll printout, as per final CF Return of Earnings
  • On a letterhead:
    – Reason for the transfer
